About Help Me Grow
DC Health created Help Me Grow as a resource to connect expectant parents and families with children 5 years and under to resources in the District. The Home Visiting Council’s C-Intake subcommittee has worked to incorporate home visiting referrals in Help Me Grow.
Parents and caregivers can self-refer by calling 1-800-MOM-BABY to talk to a Help Me Grow DC team member.

Universal Referral Form
The DC Health - Help Me Grow Universal Referral Form refers expectant parents and families of children from zero to age five to selected home visiting programs based on eligibility factors and individual needs. All program services are voluntary and provided free of charge. When a referral is received, a Help Me Grow Care Coordinator will contact the family and match them with the most appropriate home visiting program.
This form is to be completed by a provider. For example: a home visitor, a medical professional, childcare provider, MCO, community-based organizations, etc.
